I loosened it to adjust the angle where the head clamps around the shaft maybe didnt tighten enough
the machine is a bit out of balance cos it has no lower guard to cut the strings the same length
it was vibrating a bit and must have worked loose and pulled away from the shaft pulling the drive cable out with it
